***, CSAC or LCASA Preferred*** ***THIS POSITION WILL BE RESPONSIBLE FOR EVENING GROUP COVERAGE***The PORT Health Services Outpatient Unit's primary purpose is to provide substance abuse, mental health and psychiatric services to individuals. Voluntary and involuntary clients are accepted. Services available to those in need include but are not limited to screening, assessment, intake, treatment, referral, psychiatric evaluation, group therapy, family intervention, community consultation, aftercare, and follow-up supports. Additionally, the facility provides a safe, supportive, therapeutic environment. Primary Purpose of Position: The primary purpose of the Outpatient Qualified Professional is to provide screening, evaluation, assessment, treatment, after-care, and referral services to adults/children in need of outpatient treatment. To provide stabilization and treatment services to mental health and substance abusing clients. The employee will work closely with the physician, other clinical staff, and community agencies. Another function of this position is to receive referring information from referral sources and relay that information to the treatment team during weekly clinical staff meetings. The employee will provide services to clients, which include but are not limited to co-facilitating/facilitating psycho education groups, psychotherapy groups, establishing client aftercare plans, and initiating contact with family members. This position will maintain accurate and up to date referral sources, and maintain a therapeutic environment and promote appropriate and healthy interpersonal behavior. Work Schedule: This position will maintain an average work schedule of 40 hours per week. Generally, working the hours between 8:00am-9:00pm, but flexibility will be expected depending on client and Program's need. In the event of inclement weather, the employee may be required to remain on the unit beyond their normal eight-hour workday. Primary Purpose of the Position: The primary purpose of the SAIOP Qualified Professional is to provide treatment, after-care, and referral services to adults/children in need of Substance Abuse Intensive Outpatient treatment. The employee will work closely with the Program manager, Physician, other clinical staff, and community agencies. The employee will provide services to persons served which include, but are not limited to: -Individual/Group counseling and support, -family counseling, training or support, biochemical assays to identify recent drug use, strategies for relapse prevention to include community and social support systems in treatment, -Life skills, Crisis contingency planning, Disease management, Treatment support activities that have been adapted or specifically designed for persons with physical disabilities, or persons with co-occurring disorders of mental illness and substance abuse/developmental disability and substance abuse/dependence. -"First Responder " Crisis response 24/7/365 basis to recipients receiving this service. Includes face-to-face and also telephonically at all times, with the capacity for face-to-face emergency response within 2 hours The SAIOP Qualified Professional is clinically supervised by the SAIOP Program Manager. SAIOP Program Documentation Requirements: Minimum standard is a daily full service note for each day of SAIOP that includes the person's served name, Medicaid Identification number, date of service, purpose of contact, describes the provider's interventions, the time spent performing the intervention, the effectiveness of the interventions, and the signature and credentials of the staff providing the service. A documented discharge plan will be discussed with the person served and included in the record. D. Staffing Requirements: Persons who meet the requirements specified for CCS, LCAS, LCAS-A, and CSAC under Article 5C may deliver SAIOP. The program must be under the clinical supervision of a CCS or a LCAS who is on site a minimum of 50% of the hours the service is in operation. Services may also be provided by staff who meet the requirements specified for QP or AP status for Substance Abuse according to 10A NCAC 27G .0104, under the supervision of a LCAS or CCS. Individuals shall register with the Board prior to providing professional services. The maximum face-to-face staff-to-beneficiary ratio is not more than 12 adult beneficiaries to 1 QP based on an average daily attendance. The ratio for adolescents shall be 1:6. Paraprofessional level providers who meet the requirements for Paraprofessional status according to 10A NCAC 27G .0104 and who have the knowledge, skills, and abilities required for the population and age to be services may deliver SAIOP, under the supervision of a LCAS or CCS
