Duke Hospital seeks to hire a Clinical Service Nurse - Pulmonary
Hypertension who will embrace our mission of Advancing Health
Together. General Description The pulmonary hypertension nurse will
support our pulmonary providers in Clinic 2F2G. This nurse
independently plans and provides professional nursing care for
patients in accordance with physician orders and established
policies and procedures. Provides nursing services to patients and
families in accordance with the scope of the RN as defined by the
North Carolina Board of Nursing. Collaborates with members of the
health care team to enhance patient care by developing, supporting
and educating nursing staff. Makes clinical judgments based upon
current research, concepts, principles, theories, and outcomes to
solve complex problems. The heart failure nurse is an integral
member of a larger interdisciplinary team contributing to
excellence in patient care, research, teaching, and in providing
leadership to the organization. Address: Duke Clinic, 40 Duke
Medicine Cir, Clinic 2F/2G, Durham, NC Schedule: Monday-Friday - 8
hrs. shifts Duties and Responsibilities - Education of staff,
patients and caregivers - Assist with transitions of care as it
relates to those patients at high risk for readmission - Assist
with telephone follow-up for patients within 24-48 hours of
discharge, or as needed. - Inbasket messaging and response to
patient messages - Assist with triaging patient calls based on
protocols and skill set. - Assist with communication with providers
and other disciplines as needed. - Assist with necessary form
completion such as disability, FMLA, prior approvals, return to
work - Assist with data collection and quality improvement - Assist
with scheduling of outpatient procedures - Communication with
patient and/or family regarding treatment response - Communication
with referring providers and other specialty services as needed -
Completion of medication refills per Duke Nursing policy and
protocols - Develop a comprehensive nursing knowledge base of
pulmonary hypertension Knowledge and Skills - Cardiology/Pulmonary
experience preferred - Ability to lead others and work well in a
team environment - Strong customer service skills - Strong conflict
resolutions skills - Desire to learn new skills/grow skillset -
Ability to think proactively and respond professionally - Ability
to assess nursing needs of acute and chronically ill patients and
their families - Ability to independently seek out resources and
work collaboratively - Ability to communicate clearly with
patients, families, visitors, healthcare team, physicians,
administrators, leadership and others - Ability to teach patients
and families in accordance with the nursing plan of care - Ability
to document and communicate pertinent information using computer
and/or paper documentation tools Experience: - Twelve months of
appropriate clinical experience is required. Education, Degrees,
Licensure, and/or Certification: - Graduation from an accredited
Bachelor’s Degree in Nursing (or higher), Associate's Degree in
Nursing or Nursing Diploma program is required. - All registered
nurses without a Bachelor's degree in Nursing (or higher) will be
required to enroll in an appropriate BSN program within two years
of their start date and to complete the program within seven years
of their start date - Nurses hired at Duke Health with a valid
compact license are required to obtain their North Carolina nursing
license within 90 days of their start date. - BLS required.
